DNA Sample Instructions


Please follow the steps below to prepare and send your personalized kit specimen.

Items Required

For a hair sample

2 Envelopes

New envelopes.

1 Ziploc Bag

A new Ziploc plastic bag.

1 Sheet of Paper

A clean sheet of printer paper.

1 Post-it

3x3 Inch clean piece or paper or sticky pad note.

Scissors

To obtain hair. Use with caution!

HAIR SAMPLE

  1. Wash hands. Cut a pencil width - at least 1 inch long size amount of hair, or pull 20 strands from the root.

    HAIR QUANTITY: The hair image sample below notes the minimal amount of hair that can be sent. More is better.

    NOTE: Try to use hair that does not have hair conditioner on it. If one has used hair conditioner, rinse the hair, let it dry, before cutting it.

  2. Place in new unused zip lock bag.

  3. Write your name three times on the left side of a clean, 3x3 inch piece of paper as shown in image below. Sticky pad notes work well. Note: The size is important!

  4. Send items FedEx or Priority Mail to expedite and guarantee delivery. Collect your postal tracking number. Place the name paper and envelope with hair sample into a second envelope.
    Please request a waterproof envelope from FedEx or Priority mail to protect your sample.

    Please do not select the option to sign for delivery.

  5. If you are new patient, please complete the New Client Form. If you prefer the send a printed version instead of digital, include this in the envelope.

  6. Inform us once your sample is in the mail. Please include the estimated time of arrival.

OPTIONAL BLOOD SAMPLE

Please note, the blood sample is not required however, it can be beneficial for treating chronic illness.

April will inform you if the blood sample is required in your first appointment. If you would like to go ahead and send it along with the hair sample, please feel free to do that.

  1. Wash hands. Create a sterile surface with a clean sheet of printer paper on a table. Place 3 filters on top of each other to absorb excess blood. Prick finger and squeeze repeatedly.

  2. Place 4 to 15 drops of blood in different places on unbleached (brown) coffee filter. More blood is always better!

  3. Allow blood sample to dry thoroughly. Vital that DNA sample dries completely so it cannot grow bacteria.

  4. When dry, fold filter in half and place in new envelope. Seal envelope. Fold envelope and place in Ziplock bag. Do
    NOT
    seal the Ziplock bag.

  5. Write your name three times on the left side of a clean, 3x3 inch piece of paper as shown in the image below. Sticky pad notes work well. Note: The size is important!
